Most joint and back pain does not need a specialist, an MRI, or surgery. It needs an accurate diagnosis and a plan someone follows up on.
Joint, back, and other musculoskeletal complaints are evaluated at IM Clinic through history and examination, with imaging ordered when it will change the plan. Most cases are managed with activity modification, physical therapy, and appropriate medication, with referral to orthopedics, rheumatology, or pain management when warranted.
What we see most
Low back pain, knee pain, shoulder pain, neck pain, hip pain, tendinitis, and the general aching that arrives with osteoarthritis. Also the joint pain that turns out to be something systemic: inflammatory arthritis, gout, or a symptom of another condition entirely. Sorting which is which is the first job.
Examination before imaging
Imaging is genuinely useful in specific situations and genuinely unhelpful in many others. Early MRI for uncomplicated low back pain frequently finds incidental changes that are common in people without any pain at all, which leads to worry and sometimes to procedures that do not help. We image when there are red flags or when the result will change what we do.
What actually helps
For most musculoskeletal pain, the evidence favors staying active within tolerance, targeted physical therapy, and time. Topical and oral medications have a role, chosen with attention to your kidney function, blood pressure, and other conditions. For select patients, an in-office joint injection is an option for the knee, shoulder, or hip bursa. Sleep, weight, and strength all influence how much pain you experience, and they are worth addressing directly rather than treating as side issues.
Looking for the systemic causes
Joint pain with swelling in multiple joints, prolonged morning stiffness, fever, rash, or unexplained weight loss suggests something beyond wear and tear. As internists, this is squarely our territory. We test for inflammatory and autoimmune causes when the pattern calls for it, rather than treating everything as mechanical.
When we refer
Orthopedics for structural problems and surgical questions, rheumatology for inflammatory or autoimmune disease, physical therapy early and often, and pain management for complex chronic pain. We coordinate the referral and stay involved rather than handing you off.

Common questions
Do I need an MRI for my back pain?
Usually not, at least not initially. Most low back pain improves without imaging, and early scans often find changes that are common in pain-free people. We image when there are warning signs or when the result would change the plan.
Should I see an orthopedist instead?
For most joint and back pain, starting in primary care is appropriate and faster. We refer to orthopedics when there is a structural problem or a surgical question.
Is it safe to keep exercising?
For most musculoskeletal pain, staying active within tolerance is part of the treatment rather than a risk. We will tell you what to modify and what to keep doing.
Can you handle this at a same-day visit?
